I hereby consent to & authorize Beauty & Opulence Makeup Artistry (makeup artist) to perform makeup application(s), at my discretion, from this date forward. I've voluntarily elected to have this service performed after the nature & purpose of this application had been explained to me.

In the event that I may have additional questions or concerns regarding my makeup application, I will contact Juavona Johnson of Beauty & Opulence Makeup Artistry immediately.

I have, to the best of my knowledge, given an accurate account of all known allergies.

I will not hold, Juavona Johnson of Beauty & Opulence Makeup Artistry, responsible for any of my skin conditions that were present, but not disclosed at the time of the makeup application(s), which may be affected by the service(s) performed.
Media Release Authorization:

The client agrees that Juavona Johnson of Beauty & Opulence Makeup Artistry may use any & all images & /or video that is captured & recorded at the conclusion of your visits. The client agrees to release any & all claims regarding the use of these images or video, used solely for the purpose of marketing in both print & electronic format.
